Update Trainer Course Prerequisite

OSHA Requirements Prior to Registration

If you are applying to attend a trainer or update trainer course, you must complete the prerequisite verification form before you are eligible to register for the course.

Please read all information provided below before making a selection.

It is recommended that you use a PDF fill and sign application to complete the form (recommend free version of Adobe DC Reader, uncheck options before downloading – unless you need the options)

  • Download and save the Prerequisite Verification Form to your computer using the appropriate link based on the trainer course you are applying to attend (remember where the file was saved)
  • Complete and Sign the prereq verification form using the Adobe DC Reader
  • Save the completed and signed prereq verification form to your computer
  • Select Submit on the form, next to the signature block
  • Attach all required supporting documentation

A member of the PreReq Review Team will contact you within 10 business days of receipt of the prereq form:

  • If the prereq verification form is approved, you will receive the registration link for the training you are approved to attend along with the corresponding coupon code

  • If the prereq verification form cannot be approved, you will receive an email explanation for the denial
